• Change of Address Notification for FID Card or LTC

Residents should use these forms to apply for or renew an LTC or FID card with the Somerville Police Department.

New Applicant Procedures

Submit Applications By:

• Mail or drop off (mail slot) to:
  Somerville Police Substation
  1114 Broadway, Somerville, MA 02144

Packet Requirements: Must be submitted in a 9 x 12 manila envelope and include:

1. Fully completed and signed application (include Social Security number)
2. Email address written on the top of the first page
3. Copy of safety certificate
4. Copy of Massachusetts driver’s license or state-issued ID
5. Money order for $100, payable to the City of Somerville (no cash or personal checks)
6. Proof of residency (one of the following):
• Utility or credit card bill with address (redact financial info)
• Rental/Lease agreement
• Voter registration card
7. For naturalized citizens, include Naturalization Certificate (with date, number, and location of naturalization)

Next Steps:

• After review, complete applications will be scheduled for fingerprinting at the Police Substation, 1114 Broadway.
• Incomplete packets will be returned.
• Bring valid identification to your fingerprinting appointment.

Renewal Applicant Procedures

Submit Renewal Packets By:

• Mail or drop off to:

Packet Requirements: Must be in a 9 x 12 manila envelope and include:

1. Fully completed and signed application
2. Email address on top of first page
3. Copy of safety certificate (if renewing for the first time with Somerville PD)
4. Copy of Massachusetts driver’s license
5. Copy of LTC or FID (if renewing for the first time with Somerville PD)
6. Money order for $100, payable to the City of Somerville (no cash or personal checks). Applicants over 70 years old are exempt from fees
7. Proof of residency (one of the following):
• Utility or credit card bill (redact financial info)
• Rental/Lease agreement
• Voter registration card
